To avoid invalidation of the electronic application form, please read the following instructions carefully before filling up the form.

• Do not use the ENTER KEY to move from one field to another, instead use the TAB KEY
• After filling the relevant details, please click on the check box to accept the DECLARATION.
• After completing the application form, please click on the SUBMIT button.
• To avoid rejection, DO NOT fill more than one form for one applicant.
• All fields are mandatory. Incomplete forms will not be accepted by the system.
• Please enter your email Id and mobile number correctly to receive regular updates.
• Each form will be issued a receipt and a unique application number after the acceptance of the form. Please take a printout   of the receipt. This is a record of your application and will be required for any inquiries or correspondence.

• A nominal amount will be charged as online application fee through Paytm / ICICI gateway.
• Uploading of documents is mandatory.

Final admission is subject to verification of the following documents (in original) and photocopies of the same must be submitted at the time of verification of documents.

• • Passport size photos – Child, Father and Mother (mandatory)
  • Birth certificate of child
  • Proof of residence
  • In case of SC/ST/OBC certificate from the authorized authorities.
  • Adoption Deed in case of adopted child
  • Immunization Card of the child
  • Medical Certificate from a government hospital or a report of diagnosis from a recognized hospital. (In case of Special Need or Disability.)
  • Sibling proof
  • Alumni proof
  • Highest Educational Qualification Certificates of both the parents.
  • Proof of employment of the parents.
  • Photo Identity proof of parents.
  • Aadhaar Card of the Student.
  • For single parent – Legal papers of separation/custody document.
  • Report Card of the previous and current year.
  • PEN – Permanent Education Number to be brought from previous school.

TRANSPORT: – Areas Covered by the School Bus for Pre-Primary and Class I and above.

IMPORTANT NOTE

• Information regarding the assessment/verification of documents will be sent to you through email/SMS shortly after the process of registration has been completed successfully.
• The printout of the Registration Slip and the Registration Form (duly filled) with the required documents listed above (original and photocopies), should be brought by the parents at the time of verification.
• Registration fee of Rs. 1500/- will be charged for the application.
• The registration fee will be non-refundable

CSI follows a comprehensive and transparent admission process to ensure equal opportunity for all our prospective students.
